Everyone Needs:

Below are the documents that must be completed and uploaded:

• Acknowledgement of Standards of Conduct (.pdf)

• Consent and Release Agreement (.pdf)

• Letter of Health Disclosure (.pdf)

• Confidential Health Form II (.pdf)  *Must be completed by a doctor

• Parent Permission to Travel (Not necessary until March/April)

You May Also Need:

If only one parent has sole custody:
     California Residents: Affidavit of Single Parent Custody CA version (.pdf)
     Residents of any other state: Affidavit of Single Parent Custody (.pdf)
 

If you are 18 or older:
Release of Health Information (.pdf)
