<font face="Arial, serif"><b>Introduction</b></font></p>
<p style="line-height: 100%; margin-bottom: 0in">
<font face="Arial, serif">This describes the agreement between the Chicago North Chapter of AMIGOS de las Am&eacute;ricas and the volunteer and volunteer&rsquo;s family to provide the cost of the volunteer&rsquo;s training, travel, and support for the program year 2012-2013. The volunteer and the volunteer&rsquo;s parent/guardian should read and understand this agreement and its requirements prior to signing and returning the last page with the volunteer&rsquo;s application for the program at the time of scheduled interviews on </font><font face="Arial, serif"><b>November 4, 2012</b></font><font face="Arial, serif">.</font></p>

<font face="Arial, serif"><b>Program Cost</b></font></p>
<p style="line-height: 0.2in; margin-bottom: 0in">
<font face="Arial, serif">A </font><font face="Arial, serif"><b>family payment obligation of $3075 </b></font><font face="Arial, serif">along with required group fundraising activities</font><font face="Arial, serif"> cover the program cost. The program cost includes training materials, training sessions/events/retreats, domestic air fare from Chicago to the gateway city (Miami or Houston); international airfare, food, lodging and transportation in Latin America, project-related supplies, veteran supervisory Project Staff, 24-hour emergency On-Call Communications System during the summer program, and supplemental short-term international medical insurance while in Latin America. </font><font face="Arial, serif"><b>The program cost does not include: </b></font><font face="Arial, serif">a physical exam by the volunteer&rsquo;s doctor; any required immunizations; passport fee; visa fee if required; backpack, cot, mosquito net and water purification tablets; 200 postage stamps for letter writing fund raising; stationery and postage for thank you notes, or any incidentals the volunteer chooses to purchase. </font></p>

<font face="Arial, serif"><b>Family Payment Obligation</b></font></p>
<p style="line-height: 100%; margin-bottom: 0in">
<font face="Arial, serif">The family payment obligation is comprised of the </font><font face="Arial, serif"><b>non-refundable</b></font><font face="Arial, serif"> </font><font face="Arial, serif"><b>application</b></font><font face="Arial, serif"> </font><font face="Arial, serif"><b>fee of $75 </b></font><font face="Arial, serif">and the </font><font face="Arial, serif"><b>family fee of $3000</b></font><font face="Arial, serif">.</font></p>

<font face="Arial, serif"><b>Payment Schedule: </b></font><font face="Arial, serif">The Chapter is billed monthly by the AMIGOS International Office for the field program. Therefore, the Chapter collects application fees and family fees in the amounts and at the times specified in the schedule below. The application fee is submitted with the application. The family fee payments are mailed to the Treasurer at: Amigos Chicago North Chapter, PO Box 366, Winnetka, IL 60093. The AMIGOS de las Am&eacute;ricas International Office will not issue air tickets unless family fee payments are current. </font><font face="Arial, serif"><b>A portion of the family fee related to direct expenses for travel and lodging ($1840) may be tax deductible; please consult your tax professional.</b></font></p>

<font face="Arial, serif"><b>Financial Aid: </b></font><font face="Arial, serif">The International Office offers limited need-based financial aid to volunteers. Aid is available only to help defray the family fee; therefore, participation in all fundraising activities is required. There are no full scholarships. The instructions for financial aid are in the AMIGOS application package. The Chapter Paperwork Coordinator must receive all financial aid documents by </font><font face="Arial, serif"><b>December 1, 2012</b></font><font face="Arial, serif">. Veteran volunteers receive a $350 scholarship from the International Office. Scholarship awards are applied to last payment in the schedule below.</font></p>

<p style="line-height: 100%; margin-bottom: 0in">
<font face="Arial, serif"><b>Withdrawal or Dismissal Prior to Travel: </b></font><font face="Arial, serif">If a volunteer must withdraw from the program, a written withdrawal should be submitted to the Chapter President. A volunteer who </font><font face="Arial, serif"><b>withdraws </b></font><font face="Arial, serif">or is </font><font face="Arial, serif"><b>dismissed </b></font><font face="Arial, serif">from the Chapter</font><font face="Arial, serif"><b> </b></font><font face="Arial, serif">(for a rule violation at any AMIGOS sponsored event, or for failure to meet training or fundraising requirements)</font><font face="Arial, serif"><b> </b></font><font face="Arial, serif">receives a refund of the family fees paid according to the schedule below. Donations to the Chapter resulting from a volunteer&rsquo;s efforts are not refundable. </font></p>

<font face="Arial, serif"><b>Premature Return from the Field: </b></font><font face="Arial, serif">The AMIGOS International Office charges a </font><font face="Arial, serif"><b>$500 penalty </b></font><font face="Arial, serif">for each volunteer returned home from the field because of a </font><font face="Arial, serif"><b>rule violation</b></font><font face="Arial, serif">, as described in the &ldquo;AMIGOS Standards of Personal Conduct and Community Behavior&rdquo; (which is provided to each volunteer). Families of these volunteers are required to reimburse the Chapter the $500 penalty and other expenses paid by the Chapter because of the volunteer&rsquo;s premature return from the field.</font></p>

<font face="Arial, serif"><b>Group Fundraising Obligation</b></font></p>
<p align="justify" style="line-height: 100%; margin-bottom: 0in">
<font face="Arial, serif">Based on a group of 25 volunteers, group fundraising requirements equal </font><font face="Arial, serif"><b>$75,625. </b></font></p>
<p align="justify" style="line-height: 100%; margin-bottom: 0in">
<font face="Arial, serif"><b>Full participation in all fundraising activities is</b></font><font face="Arial, serif"> </font><font face="Arial, serif"><b>required</b></font><font face="Arial, serif">. AMIGOS de las Am&eacute;ricas advocates the concept that fundraising develops skills useful in the field and that group fundraising furthers the goals of, and commitment to, the program. Group fundraising also keeps the family fee as low as possible and makes participation in the Chapter more widely accessible. </font></p>

<font face="Arial, serif"><b>Fundraising Activities: </b></font><font face="Arial, serif">The Chapter uses the sale of pizza coupons, a letter-writing campaign, and a Spring event (planned and executed by current program participants) to raise funds. The success of this approach requires </font><font face="Arial, serif"><b>the good-faith effort </b></font><font face="Arial, serif">of each volunteer/family. Chapter experience demonstrates that a good-faith effort consists of each volunteer selling 30 pizza coupons, sending personalized, Chapter-approved letters to 100 households of potential donors, and participating in all phases of the planning and execution of the Spring fundraising event. </font></p>

<font face="Arial, serif"><i>Note: Volunteers may sell additional pizza coupons over the required 30 and reduce their Family Fee by the amount of income generated in this way. </i></font></p>

<p style="line-height: 100%; margin-bottom: 0in">
<font face="Arial, serif"><b>Special Arrangements: </b></font><font face="Arial, serif">The Chapter recognizes that special family circumstances may require adjustments to the payment schedule. To request special arrangements, the volunteer must notify the Chapter Treasurer in writing by December 1, 2012.</font></p>
<p style="line-height: 100%; margin-bottom: 0in">
&nbsp;</p>
<p style="line-height: 100%; margin-bottom: 0in">
<font face="Arial, serif"><b>Shortfall: </b></font><font face="Arial, serif">If group activities </font><font face="Arial, serif"><b>do not meet </b></font><font face="Arial, serif">the Chapter fundraising obligation by May 1, 2013</font><font face="Arial, serif"><i>, </i></font><font face="Arial, serif">all families shall equally contribute the shortfall amount. This </font><font face="Arial, serif"><b>payment will be due</b></font><font face="Arial, serif"> </font><font face="Arial, serif"><b>May 15, 2013</b></font><font face="Arial, serif"> and </font><font face="Arial, serif"><b>will not exceed $200 per volunteer</b></font><font face="Arial, serif">; it is not tax deductible. </font></p>
