Below are the documents that must be completed and uploaded:

• Acknowledgement of Standards of Conduct (.pdf)
• Consent and Release Agreement (.pdf)
• Letter of Health Disclosure (.pdf)
• Confidential Health Form II (.pdf)  *Must be completed by a doctor
• Parent Permission to Travel (Not necessary until March/April)

Only necessary for applicants 18 or older:

• Release of Health Information (.pdf)
